Dictation software
You speak, it types, wherever your cursor is. It works in your EHR, in a letter, in an email. You keep control of every word. Whisper, Speecher, Wispr Flow, Philips SpeechLive and the operating-system tools sit here. On-device options in this group transmit nothing at all.
Ambient AI scribe
It listens to the whole consultation and drafts the note. It removes more work and costs more every month. Freed, Heidi Health, Suki and Dragon Copilot sit here. All of them process the consultation on a server, so a signed BAA is not optional for a US practice.
Whisper is a dictation tool. It is not an ambient scribe and it does not listen to your consultations. If what you want is a note written for you from a recorded visit, one of the scribes above is the right purchase, and we would rather say that here than sell you the wrong thing.

Nobody on this page is "HIPAA compliant software"
HIPAA obligations attach to the covered entity, not to a product. Compliance is a property of your whole workflow: your devices, your access controls, your staff training, your agreements. A vendor can support that workflow. No vendor can supply it, and any page that tells you otherwise is selling you something.
What genuinely differs is the architecture, and it is a single question: does the patient audio leave the room?
- On-device. Whisper runs the model on the clinician's machine. No audio and no transcript is transmitted, so there is no business associate in the transcription path and no vendor breach that can expose that PHI. You still have to secure the machine.

Questions practices actually ask
What is the best medical dictation software in 2026?
It depends on which job you are buying for, and the two jobs are different. If you want a keyboard replacement that types what you say into your EHR, a chart, or a letter, pick a dictation tool. Whisper is our pick there, because it transcribes entirely on the clinician's own machine, so no audio and no transcript is transmitted at all, and it costs $29 a year for the first seat plus $15 a year for each additional seat, with no auto-renewal. If instead you want software that listens to the whole consultation and writes the note for you, that is an ambient scribe, and Freed, Heidi Health and Suki are built for that. An ambient scribe sends the consultation to a server. That is the trade you are making.
Which medical dictation tools are actually HIPAA compliant?
No software is HIPAA compliant on its own. HIPAA applies to the covered entity, and compliance is a property of your whole workflow, not of a single product. What differs between tools is the architecture. A cloud tool transmits Protected Health Information to a vendor, so you need a signed Business Associate Agreement with that vendor and you inherit their breach exposure. A fully on-device tool such as Whisper never transmits the audio or the transcript, so there is no business associate in the transcription path to sign an agreement with. Both routes can sit inside a compliant practice. Only one of them removes the vendor from the risk surface.
What happened to Dragon Medical and Dragon NaturallySpeaking?
Microsoft completed its acquisition of Nuance in 2022. On 8 September 2026 the old Dragon Professional product page on nuance.com returned a 301 redirect to microsoft.com/health-solutions. Microsoft now markets Dragon Copilot and offers migration from Dragon Medical One. No price is published on that page. If you are budgeting for a small practice, that alone matters: you cannot cost the incumbent without contacting sales.
Is an ambient AI scribe better than dictation for a small practice?
An ambient scribe removes more typing, and it costs more per clinician per month, every month. Freed publishes $39 to $119 per month. Mentalyc, Upheal and Blueprint publish comparable per-seat or per-session rates for therapy. Dictation software costs less and leaves you in control of the wording, which some clinicians prefer for medico-legal reasons. A practical middle path is dictation for letters, referrals and your own notes, and an ambient scribe only for the consultation types where it clearly pays for itself.
Can dictation software work without an internet connection?
Some can. Whisper runs the speech model on your own computer, so it works in a basement clinic, on a ward with no signal, and on a machine with the network disabled. Speecher also runs locally. Cloud tools such as Otter.ai, Rev, Wispr Flow and the ambient scribes need a connection, because the transcription happens on their servers.
What does medical dictation software cost for a whole practice?
Compare the three-year number, not the monthly one. Ten clinicians on a $79 per month subscription is $9,480 a year and $28,440 over three years. Ten seats of Whisper is $164 a year, or $492 over three years. That gap is the entire commercial argument for on-device dictation, and it is why we publish the seat table on this page rather than hiding it behind a sales call.
